Accepted Insurance
We accept almost all major insurance plans, including Medicare and Medicaid. Please bring your insurance card with you so that we can make a copy for your medical records. If you have a change in insurance, address or phone number, be sure to notify the receptionist so that the change can be entered into our system.
The clinic will verify insurance at every visit so please bring your information each time. Please note that individual policies differ and policies constantly change, so it is important for patients to contact their insurance companies to check on coverage.
All balances are expected to be paid at the time of service and we accept all major credit cards for payment.
Private pay patients, please ask the receptionist about discounts available for prompt payment. If you need to schedule a payment plan, please contact us before scheduling your appointment so we can best serve you.
